Where are you located?

We are located in Florida. Currently we are seeing patients virtually with telemedicine. We will be setting up an in person office in Orlando, FL  in the near future. We will give you updates on our location as soon as possible. 

Do you accept insurance?

As of this moment we are not in network with insurance, however, we are working on getting contracts with insurance companies. This can take up to 6 months. Insurance that we plan to accept include Oscar, BCBS, Aetna, Cigna, United, Medicare, and Tricare. We will give you an update as soon as we become in network.  

If I have been Grace's patient in the past, would I be considered a new patient for Summit Primary Care?

Yes. Because this is a new practice, you would need to establish care as a new patient. We will need to input all of your medical history into our system as well as get prior medical records. 

During a physical, can I discuss new concerns and problems?

A physical is an annual preventative visit which includes a full physical exam, discussing preventative screenings and labs, as well as reviewing immunizations. If you would like to discuss new problems during a preventative visit, we can do that, but it will be an extra fee for established patient visit new concern. This would be a two in one visit. Can I submit a claim to my insurance for reimbursement of service?

Yes, absolutely! We encourage you to submit a claim to your insurance to receive reimbursement for the services provided. We will provide you with superbill and other documentation needed for you to submit a claim directly to your insurance. Click here for instructions.   

Will you have a lab and imaging center on site?

No, we will not have a lab or imaging center on site. During your visit, you will be given orders to go to the diagnostic centers of your choice. Because we do not draw labs at our office, you do not need to come to your appointment fasting. 

What ages do you see?

We see  patients ages 18 and up.   

Do you have a patient portal?

Yes, we have a patient portal where you can see your records and communicate with us. You will need to download the Healow app. Click here to start the process.  

Do you have any policies or rules?

Yes, we do have several policies that we feel are important. To mention a few here, we have a 10 minute late policy, 24 hour cancelation policy, no show policy, and zero tolerance policy.
